The Västra Götaland Region has reported several deviations in connection with the introduction of the journal system Millennium last week.
In a press release, VGR writes that the deviations concern serious personal data incidents. Among other things, individual words have fallen out of the journal, resulting in a different meaning.
"A change of unknown reason in a patient journal is considered a very serious personal data incident", says Anders Andersson, information security manager, VGR, in a comment.
A report has now been filed with the Integritetsskyddsmyndigheten. The region is simultaneously opening up for the possibility that more incidents may occur as further deviations are identified.
The introduction of Millennium was paused after three days.
