Inmates have on one occasion taken themselves out on the exercise yard without staff. Cell doors, department doors, storage rooms with razors and rooms with computers and phones have all been left unlocked at least once last year, according to the Prison and Probation Service's own incident reports. Staff have also failed to lock the medicine room and cabinets with narcotics five times.

In the worst case, the staff's mistakes can lead to an inmate escaping, or barricading themselves and taking staff as hostages, says the chief safety representative Kent Ola Alperud to the radio channel.

I think it's bad.

The head of the Prison and Probation Service Conny Dahl says to P4 Västerbotten that he views the events as a failure and suggests that the problem may lie in technical issues with certain doors and new staff.
