"The introduction did not go as we thought and now we're making sure to find out what happened. Now we want to see the way forward to ensure that the Västra Götaland Region has a functioning and patient-safe healthcare information environment for the future", says the Regional Board's Chairman Helén Eliasson (S) in a press release.
According to the press release, the investigation will begin as soon as possible and a first analysis will be presented on December 10.
The Swedish Accident Investigation Authority is also considering investigating the chaotic introduction of Millennium in Västra Götaland.
It's about patient safety, says Alexander Hurtig, investigator at the Authority, to TV4.
The Accident Investigation Authority usually investigates accidents in traffic or other infrastructure, but now wants to inquire about whether any serious incidents occurred in connection with the introduction. Healthcare personnel have alerted about risks to patient safety, but it is unclear whether patients have actually been harmed.
Whether the Accident Investigation Authority will conduct a formal investigation is not yet clear.
