The introduction of the journal system Millennium could have caused deaths or seriously deteriorated health. This is stated by the Västra Götaland Region in a report to the Medical Products Agency, according to Göteborgs-Posten.

Registration was made at the end of last week and is based on events at Södra Älvsborgs hospital in Borås.

Among other things, incorrect information in journals, missing words in referral responses, and problems with acute registration occurred. According to the region, there was a risk that patients would receive the wrong dose or medication.

I take the deviations that occurred during these days seriously. That was the reason for pausing the introduction, says Jacob Wulfsberg, regional chief physician in VGR, to GP.

VGR has also made a NIS notification to the Swedish Civil Contingencies Agency (MSB), which is done in the event of an impact on a function in technical systems that has affected socially critical services.
