According to the lex Maria report that the institution submitted to the Swedish Health and Social Care Inspectorate (Ivo), deficiencies in medication management and communication may have contributed to the death.
The report also shows that the incident was reported in early April this year, writes VK.
The institution is now said to have changed its procedures, but it is not clear what changes have been made.
