The midwife was early in advocating for home births. She has been active for many years, held online courses on giving birth at home and also taken initiatives for collaborations with midwives who work with home births.
According to Ivo's decision, she has offered to be present at home births even when twins are to be born and when the baby is in a breech position, i.e. not having its head down.

"A choice she has made"
From the midwife's side, it has not been about ignorance of the guidelines, he means.

Deep-seated deficiencies
Usually, Ivo decides on a directive, where you list the deficiencies that must be rectified in a business. But here, Ivo means, it's about such deep-seated and comprehensive deficiencies that a directive is not enough. Therefore, it becomes a ban.
