Due to a technical error in the journal system Cosmic, some patients in Region Östergötland incorrectly received up to eight prescriptions for the same medication. It could have led to overmedication and the region is now being reported to Ivo, the Inspection for Care and Nursing, reports Smålands-Tidningen.
The error in the journal system, which is used by doctors to write prescriptions, was discovered in time and no patient received more than one prescription dispensed.
The incident occurred in May and is considered serious as it involved a risk of overmedication, which can be associated with danger to life.
