The patient had been treated at the hospital after overdosing on medication with a risk of death twice. The person then received medication, which was not approved by a doctor, on at least 39 different occasions.

Staff discovered that a nurse had added medication to be given as needed on a digital list, without contacting a doctor or documenting in the patient's journal.

It's about the staff not following routine, says Sara Backman Engblom, acting social manager in the municipality.

According to the report, the incorrect medication is believed to have worsened the patient's health.
