It was in April last year that the incident took place at a neuropsychiatric investigation unit in Bollnäs. Humana claims that it was a tragic accident where the 63-year-old man tripped and hit his head.
According to an investigation by TV4's Cold Facts, several signs suggest that it was a case of violence from a patient. Journals and autopsy protocols show that the man had fractures on both the front and back of his head.
The National Board of Forensic Medicine writes in its report that the injuries and circumstances indicate that the case was caused by a blow to the left eye.
According to Cold Facts, employees at the unit testify to repeated violence, but before the death, Humana not reported any violent incident at the unit to the Work Environment Authority for nine years.
After the investigation, Humana acknowledges that the reporting of incidents was mishandled at the company's unit in Bollnäs and has made a supplementary report to the Work Environment Authority about events five years back in time.
Corrected: In an earlier version of the text, there were inaccuracies about how Humana reported the death.
