"The shortcomings are serious. For a long time, the caregiver has received signals that should have led to more measures," says Peder Carlsson, head of department at Ivo, in a press release.
Among other things, Ivo assesses that Region Jönköping has not carried out sufficient risk analyses, has not carried out sufficient self-monitoring and has not acted on signals that could have affected patient safety to a sufficient degree.
Ivo's investigation into the individual pediatric cardiologist who declared the children healthy continues.
The region must now report by August 31 on what measures are being taken, Ivo requests.
