In mid-June, two girls in Alvesta were victims of an attempted murder and were seriously injured in their home. The man suspected of trying to kill the girls was found dead near the villa shortly afterwards.
According to information provided to Smålandsposten, the girls' mother had contact with social services in Alvesta municipality in close connection with the incident.
"The circumstances indicate that there may have been shortcomings in the organization's handling or implementation of interventions," the Lex Sarah report states, according to the newspaper.
The head of administration in Alvesta municipality declined to comment to the newspaper.
