According to the report to the Swedish Work Environment Authority, the incident occurred in mid-April, writes BLT.
But what was missed was performing what is called cartridge removal - to really check that there was no cartridge in the barrel.
"The result is that a shot goes through a window pane," the report states.
The text continues that "no other damage occurs along the bullet's path."
