Mohammadi suffers from heart problems and collapsed in Zanjan prison a little over a week ago. She was then taken to a local hospital.
Now, after pressure from her family and others, she has been granted a temporary conditional release on bail so that she can be transported to more advanced care in Tehran.
The foundation writes in a statement that she needs "permanent, specialized care" and that she cannot return to prison.
Mohammadi, 54, was arrested in December. By then, she had already spent much of the past ten years in prison.
She was awarded the Nobel Peace Prize in 2023 for her fight for human rights.
