Mohammadi left the hospital in Tehran on May 17, 18 days after she was admitted. She will receive medical follow-up in the coming weeks, the foundation said in a statement.
"The doctors strongly emphasize that future stressors or provocations could completely disrupt her recovery and seriously worsen her condition," the foundation writes.
Mohammadi suffers from heart problems. She was arrested in December last year but had already spent much of the past ten years in prison.
She was awarded the Nobel Peace Prize in 2023 for her fight for human rights.
