Healthcare missed stroke - patient in his 40s died after delayed diagnosis

The patient sought care after a TIA (a mild stroke) and was subsequently admitted for investigation and observation.

While in the hospital, he also showed signs of an acute stroke. However, the diagnosis was delayed because staff did not follow procedures. The patient died a few days later.

The Lex Maria report states that the patient has been subjected to serious harm in care and that the handling of the case has likely contributed to his death.
