One evening in March, a home care worker, who was not authorised to administer insulin, left it to the user - who had recently been discharged from hospital - to give himself his insulin injection, reports SVT Nyheter Öst.
No blood sugar was checked and later that evening the person had high blood sugar levels, but no nurse was called. The next day the person was found dead.
The cause of death has not been determined with certainty, but the municipality is reporting the incident to the Swedish Health and Care Inspectorate (Ivo) as a serious healthcare injury with potentially life-threatening consequences.
