A woman with a carotid artery dissection died before she could undergo surgery. According to the region, it took too long for the patient to see a specialist. A system change, unclear procedures and lack of documentation are said to have contributed to the delay.
The Lex Maria notification states that the region has now clarified the handling of referrals and strengthened administrative staffing to avoid similar incidents.
