It was only a fifth place for Simone Biles in the final of the gymnastics apparatus beam.
The gymnastics star lost her balance and fell off the beam during her routine, which made her fall out of the medal positions. Biles scored 13,100 points.
The gold was instead taken by Italian Alice D'Amato, who scored 14,366 points after her performance.
On the podium with D'Amato were Chinese Zhou Yaqin (silver) and her teammate in the Italian national team, Manila Esposito (bronze).
Later in the day, Biles had a chance for revenge when the final event in floor exercise was decided.
Then, the 27-year-old missed another gold opportunity. After 14,133 points, it was clear that it would be silver.
The winner was Rebeca Andrade from Brazil, who had been Biles' biggest challenger during the Olympic Games. The 25-year-old scored 14,166 points.
It is Andrade's second medal in the Paris Games.
The bronze was taken by Jordan Chiles, but not until after she had filed an appeal against her original score.
Simone Biles thus remains on three gold medals at the OS in Paris – in the team event, in the all-around, and in vault – and has a total of seven OS gold medals in her career. The four previous gold medals were taken by the American in Rio de Janeiro 2016.
