Hien was injured in the first half when Sweden beat Ukraine 3-1 in the playoff semi-final in Valencia on Thursday and left the team on Saturday.
At first, the national team announced that no replacement would be called in, but on Sunday a new announcement came.
"As part of the preparations for the match, national team coach Graham Potter chose to call up centre-back Victor Eriksson on Sunday," the association writes on its website.
The Hammarby defender has one senior international to his name - against Finland during the January tour in 2023. He was also selected for the friendlies against Algeria and Hungary in June 2025 but did not play.
Sweden - Poland will be played at Strawberry Arena in Solna on Tuesday with kick-off at 8:45 p.m.
