On July 13 last year, an armed man shot Donald Trump, who was then a presidential candidate, in the ear during an election meeting in Butler, Pennsylvania. One person was killed and two more were injured.

Dramatic images from the assassination attempt, where blood is flowing from Trump's ear, spread across the world.

"What happened was unforgivable and the consequences of the mistakes do not yet reflect the seriousness of the situation," it says in the report from the Senate. The report does not shed light on the perpetrator's motive, which is still unknown, but instead focuses on the security service.

"The US Secret Service failed to act on credible intelligence, failed to coordinate with local police authorities," says the committee's chairman, Republican Rand Paul.

Despite this, no one has been fired, he notes.

"We must hold individuals personally responsible and ensure that reforms are implemented fully so that it never happens again," says Paul.

The Secret Service has referred to deficiencies in communication as well as technical and human errors. Improvements regarding coordination between different police authorities and area surveillance are on the way, according to the security service. Six employees in the organization have received disciplinary penalties: relocation or pay deduction.

Trump said earlier in the week that "mistakes were made" but that he is satisfied with the investigation.
